HOW IT WORKS

- Provide us with your Name, Contact Info and Organization Name below.

- Provide us with a graphic file or high quality photo of your team logo.

-Tell us what Apparel you are interested in, we'll make a custom Pop-Up Shop for you.

-Custom Promo Code for you to share with your members.

-The promo code will include a discount and help us track team orders.

-Your Organization (or designated member) must tag and promote Evolution Hockey Apparel on social media accounts at least 2x's per month.

-We will contribute a percentage of the sales back to your organization at the end of
the season.

-Pop-Up Shop setup fee is $100 (Includes all graphic work, mock-ups, shop creation,
promo codes)
